Do I Really Need Shoulder Surgery? Insights from Dr. Ben Szerlip on ShoulderWise
At some point, nearly everyone faces a big medical decision, especially when it comes to surgery. The latest episode of ShoulderWise cuts right to the heart of one of the most common (and anxiety-filled) questions: “Do I really need surgery?” Dr. Ben Szerlip pulls back the curtain on the decision-making process and address both the medical facts and very human fears that come with considering shoulder surgery.
Take Your Time, Don’t Rush the Decision
One of the biggest insights from the episode is the myth that surgery decisions must be made immediately. Dr. Szerlip emphasizes that, contrary to what you may assume, most patients have time to process their options, gather more information, and ask plenty of questions. The idea that you only get one chance to say yes or no to surgery is, as he says, “completely bogus.” In fact, according to Dr. Szerlip, 95% of the time, taking extra time to reflect leads to better and more comfortable decisions.
Surgery Is Rarely “Necessary”And Always Personal
Dr. Szerlip points out that for most musculoskeletal issues, surgery is elective, not urgent. That doesn’t mean it’s not important, but it does mean that patients and doctors have room to weigh options. When considering whether surgery is the right path, he focuses on four main factors: an accurate diagnosis, evidence that surgery will actually solve the problem, severity and frequency of symptoms, and whether non-surgical treatments (like physical therapy) have been tried and exhausted.
The context of the individual patient matters age, activity level, and what they want from recovery are all crucial pieces. A professional athlete’s needs will differ dramatically from someone who enjoys casual golf on weekends. This approach emphasizes tailoring recommendations to each patient’s life, not applying a one-size-fits-all answer.
Addressing the Fears: Control, Recovery, and Rumors
If you’re nervous about surgery, you’re not alone. Dr. Szerlip openly acknowledges the fears most people have fear of the unknown, loss of control, pain, and concerns about recovery and finances. “A key to well-being is the perception of control,” he explains. Surgery, where you’re quite literally unconscious and handing the reins to someone else, naturally triggers anxiety.
But, he says, much of what scares us comes from misinformation and our brain’s natural negativity bias, where the one story of a “bad experience” overshadows hundreds of good outcomes. Recovery timelines, for example, are often exaggerated. With advancements in minimally invasive procedures, many patients return to work or daily activities far sooner than they expect (some within a week!). While improvements may be seen up to a year after surgery, few people are truly “out of commission” that long.
The Value of Conservative Care (And When Surgery IS the Answer)
Dr. Szerlip is candid: he’s not a “scalpel-first” surgeon. His philosophy, shaped by mentors and personal experience (including a story where he was the impatient patient), is to consider all non-operative treatments before recommending surgery. Sometimes PT and time really do work wonders, even if it takes a bit longer than you’d like.
But there are times when surgery is truly the right choice, when other treatments have failed, when symptoms are unmanageable, or when not intervening could cause further damage. In those moments, building trust is key. Dr. Szerlip focuses on walking patients through the evidence, respecting their pace, and never pressuring toward quick decisions. In some cases, he even encourages a second opinion.
Building a Partnership, Not Just a Prescription
Perhaps the greatest takeaway is Dr. Szerlip’s commitment to partnership and agency. His goal is for every patient to feel informed, empowered, and genuinely supported through these tough decisions. The true marker of great care, he says, is not just a successful outcome, but feeling like you were heard, respected, and guided with honesty and integrity.
If you’re facing your own shoulder journey or simply want to understand a bit more about the process, this episode is a must-listen for clarity, peace of mind, and practical wisdom.
